with the Practice?
All patients new to the practice are invited to attend the surgery for a general
health check with a practice nurse; they may also need to see a GP if they are
on long-term medication.The procedure includes weight, height and blood pressure
checks and allows us to summarise your medical history. This is especially useful
in case there is a delay in receiving your medical notes from your previous doctor.
Please try to bring details of past illness, vaccinations and cervical smear
history, where applicable.

My Personal Details?
If you change your name,
address or telephone number, please notify the receptionists as soon
as possible. Remember an out of date address could result in a visit
or correspondence being delayed.
